Where Homelessness Ends and Healing Begins

About

Campus

Homelessness is not one dimensional.

The solution isn't either.

Income and Skills Development

All clients at Haven for Hope are assisted in developing a person-centered income plan that’s specific to their goals, abilities, and circumstances. The mission of Income and Skills Development is to present opportunities and remove barriers to income, education, and skill development, in order to help individuals implement a strategy for long-term financial stability and permanent housing.

Transformational Case Management

Transformational Case Management provides person-centered support to individuals and families by working with each client to understand their barriers and connect them to Haven programs and other partner agencies. The Case Management teams coordinate case management planning and care with Haven’s Income and Skills Development and Housing teams.

Housing

The Housing Team helps clients to transition into housing from the Haven campus. The Housing Team works to identify and procure safe, decent, and affordable housing while assisting clients in sustaining their long-term recovery and housing.

Transformational Campus

The Transformational Campus provides shelter while clients work to address the issues that have led to their homelessness. Approximately 70 partner organizations collaborate to provide wide-ranging comprehensive services, from obtaining identification to securing permanent housing.

Unsheltered Placement Program (UPPP) and Direct Referral Program (DRP)

Veteran Services

The Veteran Services Team supports all homeless individuals residing at Haven for Hope who have ever served in the armed forces of the United States, (including National Guard and Reserve units), regardless of the length of service, or type of discharge. Services provided to Veterans include case management, counseling, housing, skills development, employment assistance, and VA benefits.

Legal Services

Legal services are provided by partners of Haven for Hope for clients experiencing homelessness.
Services include ID recovery and warrant relief.

Kennel

Haven for Hope is the only homeless service provider in San Antonio that permits individuals to bring their beloved dogs or cats with them as they access services. Our Kennel Program provides shelter, food, toys, blankets, vaccinations, and veterinary services.

Jail Outreach

While incarcerated in the Bexar County Jail, clients are referred to Haven for Hope by Bexar County Pretrial Services and the University of Health Care Services. Individuals are connected to the Jail Outreach and Transformational Support Teams, who assist clients in meeting their legal obligations, connecting to community resources, and adjusting to a new quality of life in the community.

Street Outreach

The Street Outreach Program seeks to find unsheltered individuals and families throughout the community, mostly in encampments. Street Outreach staff strive to build trust and rapport with people who are experiencing homelessness, in order to connect them, to homeless services providers including Haven for Hope.

South Campus - Low-Barrier Shelter

Haven’s South Campus is a low-barrier emergency shelter where individuals experiencing homelessness can take refuge as needed. Through our many community partners, clients on South Campus have access to three meals daily, safe indoor sleeping, shower facilities, medical care, mental health services, and all of the resources Haven offers.

Continuity of Care Transition Services

Provides wrap-around case management services to individuals who are identified as medically fragile and/or having a high need for mental health support.

Acute Care Station

The Acute Care Station (ACS) improves continuity of care for Haven for Hope clients by making on-site, after-hours acute health care services. These services are provided by San Antonio Fire Department Mobile Integrated Health Care Medics (SAFD MIH) at a designated, central location. The goal of ACS operations is to decrease the utilization of emergency room visits and triage for the most effective care of each client.
